People have really visceral feelings around these issues and we have a way to tackle them if people come up with ideas on the scale of the challenges people are experiencing.\u201d<\/p>\n<p>Health policy has quickly become a major touchstone of the Maga (\u201cMake America great again\u201d) right, as the Trump administration undertakes a <a href=\"https:\/\/www.theguardian.com\/us-news\/2025\/jan\/30\/rfk-jr-testimony-confirmation-hearing\" data-link-name=\"in body link\">shock and awe campaign<\/a> that has dramatically altered public health institutions.<\/p>\n<p>In just a few weeks in office, the administration has <a href=\"https:\/\/www.theguardian.com\/us-news\/2025\/feb\/04\/dcd-pages-trump-public-health\" data-link-name=\"in body link\">scrubbed<\/a> government health websites of information on women and racial minorities, <a href=\"https:\/\/www.theguardian.com\/us-news\/2025\/mar\/10\/alzheimers-research-trump-funding-cuts\" data-link-name=\"in body link\">reviewed billions<\/a> in scientific grant applications for conformity to the president\u2019s agenda, and confirmed the nation\u2019s <a href=\"https:\/\/www.theguardian.com\/us-news\/2025\/jan\/29\/rfk-jr-confirmation-hearing\" data-link-name=\"in body link\">foremost vaccine critic<\/a>, <a href=\"https:\/\/www.theguardian.com\/us-news\/robert-f-kennedy-jr\" data-link-name=\"in body link\">Robert F Kennedy Jr<\/a>, as the nation\u2019s top health leader at the Department of Health and Human Services. The administration has also said it will pull the US out of the World Health Organization (WHO), which it <a href=\"https:\/\/www.kff.org\/global-health-policy\/fact-sheet\/the-u-s-government-and-the-world-health-organization\/\" data-link-name=\"in body link\">helped found<\/a> in 1948.<\/p>\n<p>Additionally, congressional Republicans have floated <a href=\"https:\/\/www.theguardian.com\/us-news\/2025\/mar\/06\/gop-budget-medicare-medicaid-cuts\" data-link-name=\"in body link\">major cuts<\/a> to Medicaid, a health insurance program for the low-income and disabled that insures about 72 million Americans, to extend tax cuts that largely benefit the wealthy.<\/p>\n<p>But even outside recent upheaval, the scale of challenges to American healthcare is something to behold: the US <a href=\"https:\/\/www.theguardian.com\/us-news\/2024\/sep\/18\/american-health-system-ranks-last\" data-link-name=\"in body link\">spends<\/a> <a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022\" data-link-name=\"in body link\">more<\/a> on healthcare than almost any other country as a share of gross domestic product, yet has some of the worst outcomes among developed democracies. It is a <a href=\"https:\/\/www.commonwealthfund.org\/publications\/fund-reports\/2024\/sep\/mirror-mirror-2024#:~:text=In%20the%20U.K.%2C%20because%20services,for%20providers%20and%20patients%20alike.\" data-link-name=\"in body link\">global outlier<\/a> for failing to offer universal healthcare and one of the few countries that allows its citizens to be bankrupted by medical debt.<\/p>\n<figure id=\"70f12f63-2d26-4f31-960a-0633da53bed7\" data-spacefinder-role=\"richLink\" data-spacefinder-type=\"model.dotcomrendering.pageElements.RichLinkBlockElement\"><gu-island name=\"RichLinkComponent\" priority=\"feature\" deferuntil=\"idle\" props=\"{\"richLinkIndex\":9,\"element\":{\"_type\":\"model.dotcomrendering.pageElements.RichLinkBlockElement\",\"prefix\":\"Related: \",\"text\":\"Oz vows to make Americans healthier but dodges questions on Trump cuts\",\"elementId\":\"70f12f63-2d26-4f31-960a-0633da53bed7\",\"role\":\"richLink\",\"url\":\"https:\/\/www.theguardian.com\/us-news\/2025\/mar\/14\/mehmet-oz-senate-hearing-medicare-medicaid\"},\"ajaxUrl\":\"https:\/\/api.nextgen.guardianapps.co.uk\",\"format\":{\"design\":0,\"display\":0,\"theme\":0}}\"><\/gu-island><\/figure>\n<p>How to fix it? Don\u2019t tinker around the edges, Roy argues. Instead, look upstream for solutions to health problems. Abandon narratives about \u201cdeserving-ness\u201d. Examine what is working in cities and states.<\/p>\n<p>In an interview, Roy cited the example of the <a href=\"https:\/\/www.phillyjoybank.org\/about\" data-link-name=\"in body link\">Philadelphia Joy Bank<\/a> \u2013 a small program that provides pregnant and postpartum women with a $1,000 basic income. This money comes with no questions asked, which is a world of difference from traditional \u201cwelfare\u201d, or temporary assistance for needy families (TANF).<\/p>\n<p>TANF once provided temporary cash assistance to the poor. Since Clinton-era welfare reforms, the program has been drained of resources; its scant payments have lost venue with inflation and work requirements have saddled many with insurmountable bureaucratic barriers.<\/p>\n<p>In Connecticut, lawmakers established first-in-the-nation \u201c<a href=\"https:\/\/portal.ct.gov\/ott\/debt-management\/ct-baby-bonds\" data-link-name=\"in body link\">baby bonds<\/a>\u201d, a small investing account for each low-income child born in the state. The program provides $3,200 per child that is invested in the market, and can be used to buy a house, start a business, or pay for higher education or retirement.<\/p>\n<p>In Washington DC, reformers at the <a href=\"https:\/\/www.economicliberties.us\/press-release\/economic-liberties-launches-break-up-big-medicine-initiative-to-end-structural-conflicts-of-interest-in-healthcare\/#:~:text=Break%20Up%20Big%20Medicine%20will,take%20action%20to%20end%20conflicts\" data-link-name=\"in body link\">American Economic Liberties Project<\/a> are using the lessons of recent anti-trust victories to push for a proposed \u201cGlass-Steagall for healthcare\u201d. The initiative, called <a href=\"https:\/\/www.breakupbigmedicine.com\/\" data-link-name=\"in body link\">Break Up Big Medicine<\/a>, refers to the New Deal-era Glass-Steagall legislation that separated investment banks from commercial banks.<\/p>\n<figure id=\"6b4f8ca8-f8c2-4a49-8d25-fd36871cb7bf\" data-spacefinder-role=\"richLink\" data-spacefinder-type=\"model.dotcomrendering.pageElements.RichLinkBlockElement\"><gu-island name=\"RichLinkComponent\" priority=\"feature\" deferuntil=\"idle\" props=\"{\"richLinkIndex\":15,\"element\":{\"_type\":\"model.dotcomrendering.pageElements.RichLinkBlockElement\",\"prefix\":\"Related: \",\"text\":\"Trump\u2019s EPA aims to cut pollution rules projected to save nearly 200,000 lives: \u2018People will be hurt\u2019\",\"elementId\":\"6b4f8ca8-f8c2-4a49-8d25-fd36871cb7bf\",\"role\":\"richLink\",\"url\":\"https:\/\/www.theguardian.com\/us-news\/2025\/mar\/19\/trump-epa-pollution-regulation-cuts\"},\"ajaxUrl\":\"https:\/\/api.nextgen.guardianapps.co.uk\",\"format\":{\"design\":0,\"display\":0,\"theme\":0}}\"><\/gu-island><\/figure>\n<p>Other articles in the issue propose <a href=\"https:\/\/academic.oup.com\/healthaffairsscholar\/article\/3\/3\/qxae184\/8078918\" data-link-name=\"in body link\">home care cooperatives<\/a>, to provide better working conditions to home care workers as baby boomers age; reinvest in public hospitals and public production of pharmaceuticals, such as California\u2019s <a href=\"https:\/\/calmatters.org\/health\/2025\/01\/insulin-production-gavin-newsom\/#:~:text=Gavin%20Newsom%20in%202020%20signed,have%20fallen%20for%20other%20reasons.\" data-link-name=\"in body link\">$100m investment<\/a> in local, public insulin production; or to provide <a href=\"https:\/\/academic.oup.com\/healthaffairsscholar\/article\/3\/3\/qxae170\/8078917\" data-link-name=\"in body link\">social insurance<\/a> for times people can\u2019t work.<\/p>\n<p>\u201cPrevailing approaches to health policy are leaving people in America behind, including rural and low-income residents and people from historically marginalized communities,\u201d Kathryn A Phillips, the editor-in-chief of Health Affairs Scholar, said in a statement about the issue.<\/p>\n<p>\u201cPolicymakers need to know there is another way \u2013 an approach that prioritizes investment in patients, communities, and health care clinicians and workers.\u201d<\/p>\n<\/div>\n","protected":false},"excerpt":{"rendered":"<p>An academic journal may inject some optimism into US health policy \u2013 a scarce commodity amid the Trump administration\u2019s mass layoffs, funding freezes and the ideological research reviews.
